Armed Forces Entertainment Disclosure Form

I understand that Armed Forces Entertainment reserves the right to review all application packages for quality and content.  Packages are accepted based on quality and the requirements of our supported commands.

I understand that Armed Forces Entertainment cannot accept all entertainers that apply and that selection for a tour is not guaranteed even if my group is accepted into the program.

I understand that participation in the Armed Forces Entertainment Program is voluntary; that no talent fees or salaries are paid by Armed Forces Entertainment.

I understand that all members of the group are required to hold valid US passports to be eligible for touring.

I certify that all members of the group are 18 years of age or older.

I understand that if accepted, my group is eligible to tour for up to one year from the date of acceptance.  After one year, I must re-apply to the program.

I understand that Armed Forces Entertainment cannot return any application packages, regardless of disposition.

I understand that I am required to provide all equipment for my group (instruments, audio, lighting) excepting when Armed Forces Entertainment provides pre-positioned or rental equipment.  In the event that pre-positioned equipment is provided, Armed Forces Entertainment will only ship items that are not provided on-site (instruments except drums, effects, other items only with the approval of Armed Forces Entertainment.)

I will notify Armed Forces Entertainment of any changes to the genre, lineup and contact information for the group; failure to do so may result in removal from the Armed Forces Entertainment program.
