Armed Forces Entertainment Release Form for Artist/Performer

I understand that, during the course of Armed Forces Entertainment tours, photographs and video/music/voice recordings of the artist/performer and other members of the entertainment unit may be taken during performances, as well as during personal interaction with the troops.  In no event are entire performances to be recorded in any format by Armed Forces Entertainment. I have been advised that only performance segments may be recorded in order to provide a medium for thanking and recognizing the artist/performer, and to publicize the entertainment services provided by Armed Forces Entertainment.

I, the artist/performer, hereby give Armed Forces Entertainment permission to take, record, and use photographs, recordings, and reproductions thereof, for the limited purpose of recognizing my interaction with, and live performances before the troops, and to help promote the professional entertainment services that are provided by Armed Forces Entertainment.  

I concur and consent that any promotional materials that may be made from my Armed Forces Entertainment tour may be released in any and all media including theatricals, non-theatricals, radio, videocassette, television, electronic usage, and printed products for the specific purposes stated above. I hereby give Armed Forces Entertainment the right to approve the finished promotional product or products, the advertising copy or matter that may be used in conjunction therewith, and the use to which it may be applied without any additional approval or input from me. I further consent to the use of my name in connection with my comments and opinions.
