Armed Forces Entertainment Release Form for Artist/Performer

I understand that, during the course of Armed Forces Entertainment tours, photographs and video/music/voice recordings of the artist/performer and other members of the entertainment unit may be taken during performances, as well as during personal interaction with the troops. I have been advised that only performance segments may be recorded in order to provide a medium for thanking and recognizing the artist/performer, and to publicize the entertainment services provided by Armed Forces Entertainment.

I confirm that any and all material furnished by me for this program is either my own or otherwise authorized for such use without obligation to me or any third party. I also agree to the use of my name, likeness, portrait or pictures, voice and biographical material about the group for program or series publicity and promotional purposes. I agree that the material may be broadcast and or distributed in all manner of media to include but not limited to print, web-based, radio, and television.

I, the artist/performer, hereby freely and without restraint consent to and give unto Armed Forces Entertainment and its agents or assigns or anyone authorized by the same the unrestrained right in perpetuity to own, utilize, or alter the released subject matter in any manner the Releasees may see fit and for any purpose whatsoever, all of the foregoing to be without limitation of any kind. I hereby authorize the Releasees and grant unto them the unrestrained rights to utilize the Released Subject matter in connection with the program’s advertising, publicity, public displays, and exhibitions. I, the artist/performer give permission to Armed Forces Entertainment to take, record, and use photographs, recordings and reproductions thereof, for the purpose of recognizing my interaction with, and live performances before the troops, and to help promote the professional entertainment services that are provided by Armed Forces Entertainment.

I hereby waive any right to inspect or approve the finished photographs, advertising copy, or printed matter that may be used. I understand that, although Armed Forces Entertainment will endeavor to use my photograph, video, song, or words or likeness in accordance with standards of good judgment, Armed Forces Entertainment cannot warranty or guarantee that any further dissemination of these photograph, video, song, or words or likeness will be subject to Armed Forces Entertainment supervision or control.

I concur and consent that any promotional materials that may be made from my Armed Forces Entertainment tour may be released in any and all media including theatricals, non-theatricals, radio, videocassette, television, electronic usage, and printed products for the specific purposes stated above. I hereby give Armed Forces Entertainment the right to approve the finished promotional product or products, the advertising copy or matter that may be used in conjunction therewith, and the use to which it may be applied without any additional approval or input from me. I further consent to the use of my name in connection with my comments and opinions.
